PLEASE perform the following TO ALLOW FORMULATION OF A PERSONALIZED TREATMENT PLAN FOR YOU: 

1. A current referral letter* from your doctor if the previous referral has expired.
2. Have your blood tests done at least 1 week prior to your appointment (unless advised otherwise).
3. Have your imaging done at least 1 week prior to your appointment (unless advised otherwise).
4. Inform us of any changes in medications or major medical events at the time of your consultation.

*Note: 

• A GP and a Specialist referral is current for only 12 months and 3 months respectively.
• We will not be able to process Medicare rebates on your behalf without a current referral.
• If your referral is due to expire soon, kindly provide us with a new referral from your GP.
